All team officials require a criminal record check

1. Copy the access code 4NX4CF5GJG
2. Click on the link  Criminal Record Check
3. Paste the access code in the designated area
4. Complete the application as directed
5. Once completed, please take a screen shot and email to registrar@surreyminorhockey.com. Be on the lookout for an email from them as you may be directed to go into an attachment for further verification
6. Your Criminal Record Check will be sent directly to the association

These instructions are for parent/guardians who have not played minor hockey or volunteered on a hockey team before.  If you have volunteered or played before but are not sure what your hockey ID is please contact the registrar and provide your full name and date of birth to confirm.

Do not create a hockey ID for a player (Player ID's must go through the Registrar)

Go to https://ehockey.hockeycanada.ca/ehockey/account/login.aspx

You will need to log in and then add a participant.

You will need a hockey ID, then register for the program through BC Hockey.

For exhibition games you can request a game number through your PCAHA League Manager.  You can find your League Manager by going to the PCAHA Website, and on the right hand side of the page under Leagues, click on the appropriate league.

1. After a game, the gamesheet has several copies that should be distributed according to the PCAHA Responsibility for sending in scoresheets
2. If you are the home team, enter the gamesheet on the TeamLink website
3. Send your gamesheet to the PCAHA League Manager

Payment amounts are found on the PCAHA Referee Page, click the Referee / Linesmen Allowances link.

For Bantam A teams and above you must fill out a Referee No Show Report

To get permission to go to a tournament you need to:

In District

1. Get a permission letter from the Surrey Minor Hockey Association President
2. Send the permission letter from the SMHA president to the PCAHA League Manager
3. PCAHA League Manager will respond with a Tournament Permission letter

Interdistrict Tournaments  ( non-PCAHA associations )

1. Get a permission letter from the Surrey Minor Hockey Association President
2. Follow the PCAHA Interdistrict Travel Checklist \ Form (Form is on page 2) found for the PCAHA Forms page
3. Send the completed package to the PCAHA League Manager
4. PCAHA League Manager will respond will a Tournament Permission letter

Out of Province / USA Hockey Tournaments

1. Get a permission letter from the Surrey Minor Hockey Association President
2. Follow the PCAHA Out of Province & USA Hockey Tournament Travel / Exhibition Game Form (Form is on page 2) found for the PCAHA Forms page
3. Send the completed package to the PCAHA League Manager
4. PCAHA League Manager will respond will a Tournament Permission letter

Assign one contact from your team to email the SMHA Registrar with questions or requests with respect to your roster.   The registrar rosters all the players and team officials but you must inform your division manager of any additions to your roster.

The safety person should hold those for the season. These sheets hold personal information and should be kept private at all times. In case of emergency, the sheets should be easily accessed during games or practices.

1. Inform the PCAHA League Manager.
2. Contact our Ice Allocator.
3. If the game is within 48 hours, contact the SMHA Referee In Charge to arrange referees.

All teams are required to complete a Team Start Up Checklist to ensure their Team Officials are properly qualified for the season.
