REGISTER

Players who are transferring from another association must submit the following to registrar@surreyminorhockey.com:

• Download and complete a registration form: Registration Form
• Application for Player Movement (to be signed by your previous association BEFORE coming to Surrey Minor Hockey Association) found here: PCAHA Forms Site
• Copy of player’s Birth Certificate or passport. If not a Canadian birth certificate, provide Landed Immigrant Status, Permanent Resident Status, work permit
• 3 Proofs of current address as per PCAHA guidelines (see below for what is acceptable proof of address)

Registration is not complete until all requirements are met and player movement is approved by Pacific Coast Amateur Hockey (PCAHA). New Players who have never played hockey please see New Player tab above.

All player transfers are subject to a fee determined by PCAHA to be collected at the time of registration.

Once the player movement has been processed in full and your child's profile has been received by SMHA through the Hockey Canada Registry you will be sent an email with further instructions to use our online registration form.

Waivers: There are multiple waivers that require review and acknowledgement prior to completing the registration process. Please ensure you take the time to review and understand the waivers

Documents that are considered acceptable are the following:

•              utility bills (hydro, gas, water) – preferred (we require the                       complete first page, including pay stub
•             -cable/satellite/internet
•             -property/contents insurance
•             -property tax bill/assessment
•             -car insurance/driver’s license
•             -bank/visa statement
•             -GST statement, income tax assessment, child tax credit
•             -School registration confirming address

Players who are new to hockey will need to submit the documents listed below to registrar@surreyminorhockey.com.  Once the registrar has created your child's profile you will be emailed further instruction with a link to finalize registration.

The following requirements must be met to complete registration:

AGE:

The should have reached his/her 4th birthday by December 31 of the current season to enroll.

Parent/Guardian

A child may only be registered in by the custodial parent with whom he/she resides. This residence must be within the boundaries of Surrey Minor Hockey Association. View Boundary Map above

Documents:

• Player Registration Form
• Copy of Canadian birth certificate or passport.
• 3 Proofs of current address as per PCAHA guidelines (see below list of accepted proofs of address)

Preferred Minimum Requirements:

• Players from 5 to 8 years old highly recommended some skating experience will be helpful but not necessary
• Players 9 to 18 years old highly recommend skating experience in gear

International Players:

Permanent Resident

If your child was born OUTSIDE of Canada, you must also complete the following:

• IIHF Unlimited Transfer Form
• IIHF Letter of Approval - International Transfer
• Provide child's Permanent Resident card,
• Copy of passport
• Confirmation of primary medical insurance
• IIHF Letter from ParentsTransfers-LIMITED

International Students:

If you are an international student you will also need to complete the following:

• IIHF Unlimited Transfer Form
• IIHF Letter of Approval - International Transfer
• Copy of passport
• Copy of Study Visa/Permit
• Confirmation of school registration for current school year
• Confirmation of primary medical insurance

Please refer to our Fee Schedule. Our preferred method of payment is credit card or e-transfer at the time of registration. We no longer accept cash or cheques.

New players (skaters and goaltenders) who have never played in minor hockey (12 and older) must pay a non-refundable fee of $40 for a skating and skill assessment. If the player, for safety reasons, cannot be registered with Surrey Minor Hockey Association it may be possible for them to participate in our Hockey Start Program which will be discussed after the assessment is complete.

• Payment Schedule for the 2022-2023 season is as follows:
  • 1st Installment - At time of registration
  • 2nd Installment - June 30th
  • 3rd Installment - July 15th
  • 4th Installment - Aug 5th
• If you select to pay via credit card you are welcome to use the automated payment schedule which has been set up. Please note processing fees will be applied to your final total.
• If you choose to pay via e-transfer you can schedule payments in advance using the above dates if needed

These e-transfers are being manually processed so please allow some time for your payment to reflect on your Teamsnap invoice. Once your e-transfer has been received and processed you will be able to see the remaining balance on your invoice which can be found on your teamsnap home page when logging in using a PC.

* Registration fees may be subject to change due to Covid-19 and the AGM.

1. Late Fees a late fee of $100 will be applied to all registrations received after July 31st
2. Registration fees do not include team fees. Team fees differ from one team to another depending on a team’s activities. These fees are used to pay for tournament entries, game officials, exhibition games, extra practice ice, etc.
3. Payment must be received in full prior to your child(ren) stepping on the ice for the season. No exceptions.
4. Rep Tryout fees Early Bird Rep Tryout Fees must be received prior to July 31st. Regular Rep Tryout Fees must be received prior to Aug 10th.
5. Carding Fees carding fees for all confirmed rep players are $200 to be paid through your team.
6. 3rd Child Discount If you have 3 or more children registered with the association in the same season you are eligible for a 3rd child discount. Please submit a Refund Request form at the time of registration (the form is embedded within our online registration form for ease of access).
7. New Players (skaters and goaltenders) who have never played on a hockey team (Atom and up) must pay a non-refundable fee of $40 for a skating and skill assessment for safety purposes.

9. Returned / NSF Cheques A dishonoured payment will be subject to an immediate $35.00 service charge per incident. This includes stop payments. Failure to resolve this situation immediately is subject to the player’s registration being suspended, and the potential of not being selected to a team

Our Prep Camps are a great way to get ready for up and coming hockey evaluations.  You can register for camps through the main registration link or separately. You can register for our prep camps when completing your main registration for the season. 

All Prep Camp registrations received after July 15th may be subject be wait listed.

Prep Camps are available for the following divisions:

U8, U9, U11, U13, U15 and U18
